7e989f7062 scsi: sd_zbc: Use kvzalloc() to allocate REPORT ZONES buffer

We have two reports of failed memory allocation in btrfs' code which is
calling into report zones.

Both of these reports have the following signature coming from
__vmalloc_area_node():

 kworker/u17:5: vmalloc error: size 0, failed to allocate pages, mode:0x10dc2(GFP_KERNEL|__GFP_HIGHMEM|__GFP_NORETRY|__GFP_ZERO), nodemask=(null),cpuset=/,mems_allowed=0

Further debugging showed these where allocations of one sector (512
bytes) and at least one of the reporter's systems where low on memory,
so going through the overhead of allocating a vm area failed.

Switching the allocation from __vmalloc() to kvzalloc() avoids the
overhead of vmalloc() on small allocations and succeeds.

Note: the buffer is already freed using kvfree() so there's no need to
adjust the free path.

0ed01a4c84 scsi: aacraid: Rearrange order of struct aac_srb_unit

struct aac_srb_unit contains struct aac_srb, which contains struct sgmap,
which ends in a (currently) "fake" (1-element) flexible array.  Converting
this to a flexible array is needed so that runtime bounds checking won't
think the array is fixed size (i.e. under CONFIG_FORTIFY_SOURCE=y and/or
CONFIG_UBSAN_BOUNDS=y), as other parts of aacraid use struct sgmap as a
flexible array.

It is not legal to have a flexible array in the middle of a structure, so
it either needs to be split up or rearranged so that it is at the end of
the structure. Luckily, struct aac_srb_unit, which is exclusively
consumed/updated by aac_send_safw_bmic_cmd(), does not depend on member
ordering.

The values set in the on-stack struct aac_srb_unit instance "srbu" by the
only two callers, aac_issue_safw_bmic_identify() and
aac_get_safw_ciss_luns(), do not contain anything in srbu.srb.sgmap.sg, and
they both implicitly initialize srbu.srb.sgmap.count to 0 during
memset(). For example:

During aac_send_safw_bmic_cmd(), a separate srb is mapped into DMA, and has
srbu.srb copied into it:

        srb = fib_data(fibptr);
        memcpy(srb, &srbu->srb, sizeof(struct aac_srb));

Only then is srb.sgmap.count written and srb->sg populated:

        srb->count              = cpu_to_le32(xfer_len);

        sg64 = (struct sgmap64 *)&srb->sg;
        sg64->count             = cpu_to_le32(1);
        sg64->sg[0].addr[1]     = cpu_to_le32(upper_32_bits(addr));
        sg64->sg[0].addr[0]     = cpu_to_le32(lower_32_bits(addr));
        sg64->sg[0].count       = cpu_to_le32(xfer_len);

But this is happening in the DMA memory, not in srbu.srb. An attempt to
copy the changes back to srbu does happen:

But this was never correct: the sg64 (3 u32s) overlap of srb.sg (2 u32s)
always meant that srbu.srb would have held truncated information and any
attempt to walk srbu.srb.sg.sg based on the value of srbu.srb.sg.count
would result in attempting to parse past the end of srbu.srb.sg.sg[0] into
srbu.srb_reply.

After getting a reply from hardware, the reply is copied into
srbu.srb_reply:

        srb_reply = (struct aac_srb_reply *)fib_data(fibptr);
        memcpy(&srbu->srb_reply, srb_reply, sizeof(struct aac_srb_reply));

This has always been fixed-size, so there's no issue here. It is worth
noting that the two callers _never check_ srbu contents -- neither
srbu.srb nor srbu.srb_reply is examined. (They depend on the mapped
xfer_buf instead.)

Therefore, the ordering of members in struct aac_srb_unit does not matter,
and the flexible array member can moved to the end.

(Additionally, the two memcpy()s that update srbu could be entirely
removed as they are never consumed, but I left that as-is.)

8a4b738d45 scsi: aacraid: Fix double-free on probe failure

aac_probe_one() calls hardware-specific init functions through the
aac_driver_ident::init pointer, all of which eventually call down to
aac_init_adapter().

If aac_init_adapter() fails after allocating memory for aac_dev::queues,
it frees the memory but does not clear that member.

After the hardware-specific init function returns an error,
aac_probe_one() goes down an error path that frees the memory pointed to
by aac_dev::queues, resulting.in a double-free.

7dfc576417 scsi: lpfc: Initialize status local variable in lpfc_sli4_repost_sgl_list()

A static code analyzer tool indicates that the local variable called status
in the lpfc_sli4_repost_sgl_list() routine could be used to print garbage
uninitialized values in the routine's log message.

Fix by initializing to zero.

f94001eec8 scsi: spi: Fix sshdr use

If scsi_execute_cmd returns < 0, it doesn't initialize the sshdr, so we
shouldn't access the sshdr. If it returns 0, then the cmd executed
successfully, so there is no need to check the sshdr. This has us access
the sshdr when we get a return value > 0.

72ca994bd5 scsi: mpt3sas: Avoid IOMMU page faults on REPORT ZONES

Some firmware versions of the 9600 series SAS HBA byte-swap the REPORT
ZONES command reply buffer from ATA-ZAC devices by directly accessing the
buffer in the host memory. This does not respect the default command DMA
direction and causes IOMMU page faults on architectures with an IOMMU
enforcing write-only mappings for DMA_FROM_DEVICE DMA driection (e.g. AMD
hosts).

Avoid such issue by always mapping the buffer of REPORT ZONES commands
using DMA_BIDIRECTIONAL (read+write IOMMU mapping). This is done by
introducing the helper function _base_scsi_dma_map() and using this helper
in _base_build_sg_scmd() and _base_build_sg_scmd_ieee() instead of calling
directly scsi_dma_map().

2869e7d563 scsi: mpt3sas: Remove scsi_dma_map() error messages

When scsi_dma_map() fails by returning a sges_left value less than zero,
the amount of logging produced can be extremely high.  In a recent end-user
environment, 1200 messages per second were being sent to the log buffer.
This eventually overwhelmed the system and it stalled.

These error messages are not needed. Remove them.

3f4959ffd1 scsi: ufs: core: Fix hba->last_dme_cmd_tstamp timestamp updating logic

The ufshcd_add_delay_before_dme_cmd() always introduces a delay of
MIN_DELAY_BEFORE_DME_CMDS_US between DME commands even when it's not
required. The delay is added when the UFS host controller supplies the
quirk UFSHCD_QUIRK_DELAY_BEFORE_DME_CMDS.

Fix the logic to update hba->last_dme_cmd_tstamp to ensure subsequent DME
commands have the correct delay in the range of 0 to
MIN_DELAY_BEFORE_DME_CMDS_US.

Update the timestamp at the end of the function to ensure it captures the
latest time after any necessary delay has been applied.

The command completion was done early while aborting the commands in driver
unload path but outside lock to avoid the WARN_ON condition of performing
dma_free_attr within the lock. However this caused race condition while
command completion via multiple paths causing system crash.

Hence complete the command early in unload path but within the lock to
avoid race condition.

Link up failure is observed as a result of flash read failure.  Current
code does not check flash read return code where it relies on FW checksum
to detect the problem.

Firmware only supports single DSDs in ELS Pass-through IOCB (0x53h), sg cnt
is decided by the SCSI ML. User is not aware of the cause of an acutal
error.

Return the appropriate return code that will be decoded by API and
application and proper error message will be displayed to user.

The qedi_dbg_do_not_recover_cmd_read() function invokes sprintf() directly
on a __user pointer, which results into the crash.

To fix this issue, use a small local stack buffer for sprintf() and then
call simple_read_from_buffer(), which in turns make the copy_to_user()

There is a potential out-of-bounds access when using test_bit() on a single
word. The test_bit() and set_bit() functions operate on long values, and
when testing or setting a single word, they can exceed the word
boundary. KASAN detects this issue and produces a dump:

55c2ea0184 scsi: qla2xxx: Replace all non-returning strlcpy() with strscpy()

strlcpy() reads the entire source buffer first.  This read may exceed the
destination size limit.  This is both inefficient and can lead to linear
read overflows if a source string is not NUL-terminated [1].  In an effort
to remove strlcpy() completely [2], replace strlcpy() here with strscpy().
No return values were used, so direct replacement is safe.

As of commit 7d1d86518118 ("[SCSI] libsas: fix false positive 'device
attached' conditions"), reset the phy->entacted_sas_addr address to a
zero-address when the link rate is less than 1.5G.

Currently we find that when a new device is attached, and the link rate is
less than 1.5G, but the device type is not NO_DEVICE, for example: the link
rate is SAS_PHY_RESET_IN_PROGRESS and the device type is stp. After setting
the phy->entacted_sas_addr address to the zero address, the port will
continue to be created for the phy with the zero-address, and other phys
with the zero-address will be tried to be added to the new port:

Currently, the UIC_COMMAND_COMPL interrupt is disabled and a wmb() is used
to complete the register write before any following writes.

wmb() ensures the writes complete in that order, but completion doesn't
mean that it isn't stored in a buffer somewhere. The recommendation for
ensuring this bit has taken effect on the device is to perform a read back
to force it to make it all the way to the device. This is documented in

c547fceef6 scsi: lpfc: Update lpfc_ramp_down_queue_handler() logic

Typically when an out of resource CQE status is detected, the
lpfc_ramp_down_queue_handler() logic is called to help reduce I/O load by
reducing an sdev's queue_depth.

However, the current lpfc_rampdown_queue_depth() logic does not help reduce
queue_depth.  num_cmd_success is never updated and is always zero, which
means new_queue_depth will always be set to sdev->queue_depth.  So,
new_queue_depth = sdev->queue_depth - new_queue_depth always sets
new_queue_depth to zero.  And, scsi_change_queue_depth(sdev, 0) is
essentially a no-op.

Change the lpfc_ramp_down_queue_handler() logic to set new_queue_depth
equal to sdev->queue_depth subtracted from number of times num_rsrc_err was
incremented.  If num_rsrc_err is >= sdev->queue_depth, then set
new_queue_depth equal to 1.  Eventually, the frequency of Good_Status
frames will signal SCSI upper layer to auto increase the queue_depth back
to the driver default of 64 via scsi_handle_queue_ramp_up().

42146f9724 scsi: sd: Fix wrong zone_write_granularity value during revalidate
commit 288b3271d920c9ba949c3bab0f749f4cecc70e09 upstream.

When the sd driver revalidates host-managed SMR disks, it calls
disk_set_zoned() which changes the zone_write_granularity attribute value
to the logical block size regardless of the device type. After that, the sd
driver overwrites the value in sd_zbc_read_zone() with the physical block
size, since ZBC/ZAC requires this for host-managed disks. Between the calls
to disk_set_zoned() and sd_zbc_read_zone(), there exists a window where the
attribute shows the logical block size as the zone_write_granularity value,
which is wrong for host-managed disks. The duration of the window is from
20ms to 200ms, depending on report zone command execution time.

To avoid the wrong zone_write_granularity value between disk_set_zoned()
and sd_zbc_read_zone(), modify the value not in sd_zbc_read_zone() but
just after disk_set_zoned() call.

The myrb and myrs drivers use an odd way of implementing their sysfs files,
calling snprintf() with a fixed length of 32 bytes to print into a page
sized buffer. One of the strings is actually longer than 32 bytes, which
clang can warn about:

2ff65e968b scsi: qla2xxx: Delay I/O Abort on PCI error
commit 591c1fdf2016d118b8fbde427b796fac13f3f070 upstream.

Currently when PCI error is detected, I/O is aborted manually through the
ABORT IOCB mechanism which is not guaranteed to succeed.

Instead, wait for the OS or system to notify driver to wind down I/O
through the pci_error_handlers api.  Set eeh_busy flag to pause all traffic
and wait for I/O to drain.

The system was under memory stress where driver was not able to allocate an
SRB to carry out error recovery of cable pull.  The failure to flush causes
upper layer to start modifying scsi_cmnd.  When the system frees up some
memory, the subsequent cable pull trigger another command flush. At this
point the driver access a null pointer when attempting to DMA unmap the
SGL.

Add a check to make sure commands are flush back on session tear down to
prevent the null pointer access.

820e3ae588 scsi: qla2xxx: Split FCE|EFT trace control
commit 76a192e1a566e15365704b9f8fb3b70825f85064 upstream.

Current code combines the allocation of FCE|EFT trace buffers and enables
the features all in 1 step.

Split this step into separate steps in preparation for follow-on patch to
allow user to have a choice to enable / disable FCE trace feature.
